Food & Nutrition Counseling

Nutrition counseling is a client-centered service that allows you and your dietitian to dive deep into your health goals, relationship with food, and food preferences, to determine how you would like nutrition and movement to fit into your life. We take pride in helping our clients develop personalized, sustainable habit changes that lead to long term health benefits.  We serve a wide range of nutrition related conditions that include but are not limited to diabetes, heart disease, chronic kidney disease, non-alcoholic fatty liver disease, gastrointestinal diseases, malnutrition, food allergies and intolerances, weight management, disordered eating, pregnancy, athletic performance, picky eating, infant and toddler feeding, and general healthy diets.

You can expect your dietitian to complete an initial assessment and provide a nutrition plan that will meet you where you are at.  Individual follow-ups with your dietitian will give you the tools to help maintain a healthier lifestyle for a lifetime.  The frequency and number of follow-ups are determined by you and your dietitian at the initial assessment.

Diabetes Education and Support

We believe our American Diabetes Association accredited diabetes self-management and education program is the right fit for you.  Whether you are newly diagnosed with diabetes or have had it for years, our program is something you can turn to for answers.  You will be assisted with everything from eating decisions to reducing risks.  You will work on coping strategies to ease the burden and support you in making changes that benefit your overall health.  Here are just a few examples of what an *individual diabetes self-management and education appointment may include: ​

  • work with you to learn how to use devices like meters, insulin pens, pumps, continuous glucose monitors and more. 

  • review information from your devices and lifestyle to identify patterns and problem areas, and collaborate with you to produce workable solutions 

  • develop an education plan that fits your lifestyle and respects your habits, interests, and beliefs, plus set you up with resources for success 
    *patients will type 1 diabetes complete individual diabetes education at this time. 
     

Our group classes are tailored to assist patients with type 2 diabetes learn the most effective methods to self-manage diabetes. Our classes are team taught by our registered dietitians and registered nurses.  Class topics include: 

  • Know your numbers 

  • Type 2 diabetes meal planning 

  • Prevention of long-term complications 

  • Managing diabetes for life 

 

Every month we host a free diabetes support group held the 3rd Thursday from 12:00 – 12:45 pm at our Kirksville clinic, inside Northeast Family Health.  This is open to all who have been affected by diabetes.  Please contact us to learn more.

Medicare Weight Loss Program
Exercise and Healthy Diet

Medicare Weight Loss Program

This program uses an interdisciplinary approach between our medical providers and dietitians. The treatment uses interventions to help you change habits that have prevented weight loss.  You will work closely with your Registered Dietitian Nutritionist (RDN) in a one-on-one setting to make lifestyle modifications to promote sustainable weight loss. This is an annual benefit covered at 100% by Traditional Medicare with 22 visits per year.  We recommend clients with Medicare Advantage plans check their plan benefits to ensure it is also covered at 100% as they may have different cost shares. 

 

Your healthcare provider may suggest this program if you have decided to lose weight. He or she may also recommend it if you have been making lifestyle changes on your own.  At the initial assessment you will develop a weight management plan that is tailored to you.  You will focus on making small realistic changes because dramatic changes may work for a brief time, but they are less likely to lead to lasting weight loss.

First, you will meet with your RDN weekly for 1-2 months.  You will then transition to once every 2-4 weeks. In addition to one-on-one sessions with your RDN, you can take advantage of the monthly support group for individuals completing this program.

Lifestyle Change Program

Are you considering making healthy changes to prevent type 2 diabetes?  Our CDC-recognized diabetes prevention lifestyle change program can help you build new habits that last a lifetime. This program is also sometimes referred to as the Diabetes Prevention Program (DPP).  A lifestyle coach will lead each group to help you learn new skills, encourage you to set and reach goals, and keep you motivated.  The coach will facilitate discussions and help make the program fun and engaging.  Your group can share ideas, celebrate successes, and work to overcome obstacles.   

 

The program runs for 1 year.  During the first 6 months of the program, you will meet once every 1 - 2 weeks.  During the second 6 months, you will meet once or twice a month.  Making lifestyle changes is an ongoing process.  Staying in the program for the full year is essential to help you stick to new habits and avoid slipping back into old habits. Both in-person and virtual groups are available, starting at various times throughout the year.
