Food & Nutrition Counseling
Nutrition counseling is a client centered service that allows your dietitian and you to dive deep into your health goals, relationship with food, food preferences, and determine how you would like nutrition and movement to fit into your life. We take pride in helping our clients develop personalized, sustainable habit changes that lead to long term health benefits. We serve a wide range of nutrition related conditions that include but are not limited to diabetes, heart disease, chronic kidney disease, non-alcoholic fatty liver disease, gastrointestinal diseases, malnutrition, food allergies and intolerances, weight management, disordered eating, pregnancy, athletic performance, picky eating, infant and toddler feeding, and general healthy diets.

You can expect your dietitian to complete an initial assessment and provide a nutrition plan that will meet you where you are at. Individual follow-ups with your dietitian will give you the tools to help maintain a healthier lifestyle for a lifetime. Frequency and number of follow-ups are determined by you and your dietitian at the initial assessment.

Diabetes Education and Support
Whether you are newly diagnosed with diabetes or have had it for years, diabetes self-management education and support programs are something you can turn to for answers. You will be assisted with everything from eating decisions to reducing risks. You will work on coping strategies to ease the burden and support you in making changes that benefit your overall health. Here are just a few examples of what an individual diabetes self-management and education appointment may include:

work with you to learn how to use devices like meters, insulin pens, pumps, continuous glucose monitors and more.

review information from your devices and lifestyle to identify patterns and problem areas and collaborate with you to come up with workable solutions

you will develop a plan that fits your lifestyle and respects your habits, interest, and beliefs, and set you up with resources for success
Our program is accredited through the American Diabetes Association. Medicare and most insurance policies will cover diabetes self-management education and support. Research shows diabetes-self-management education and support can help lower your blood sugar, reducing the risk of complications and improve your overall health. There are four key times to ask your provider to refer you to a diabetes self-management education and support program.       1.) At diagnosis       2.) Annually or when not meeting treatment targets.       3.) When complicating factors develop.       4.) When a transition in life or care happens.
Medicare Weight Loss Program
This program is only for clients who see a medical provider associated with Northeast Missouri Health Council. Through this treatment you learn how to change your eating and movement habits. The treatment uses interventions to help you change habits that have prevented weight loss. You will work closely with your Registered Dietitian Nutritionist (RDN) in a one-on-one setting. You will learn to change your lifestyle to promote sustainable weight changes. This is an annual benefit covered at 100% by Medicare with 22 visits per year. We recommend clients with Medicare Advantage plans check their plan benefits to ensure it is also covered at 100%.
Your healthcare provider may suggest this program if you have decided to lose weight. He or she may also recommend it if you have been making lifestyle changes on your own. At the initial assessment you will develop a weight management plan that is tailored to you. You will focus on making small realistic changes. Dramatic changes may work for the a short time but they are less likely to lead to lasting weight loss.

At first, you will meet with your RDN weekly for 1-2 months. Then you will then transition to once every 2-4 weeks. In addition to one-on-one sessions with your RDN, you can take advantage of the monthly support group for individuals completing this program.
Lifestyle Change Program
Are you considering making healthy changes to prevent type 2 diabetes? Our CDC-recognized diabetes prevention lifestyle change program can help you build new habits that last a lifetime. A lifestyle coach will lead each group to help you learn new skills, encourage you to set and meet goals, and keep you motived. The coach will facilitate discussions and help make the program fun and engaging. With your group you can share ideas, celebrate successes, and work to overcome obstacles.
The program runs for 1 year. During the first 6 months of the program, you will meet once every 1 - 2 weeks. During the second 6 months, you will meet once or twice a month. Making lifestyle changes is an ongoing process. Staying in the program for the full year is essential to help you stick to new habits and avoid slipping back into old habits.
